QA LEVEL 2 PROFICIENCY REQUIREMENTS

  1. Verified Level 1 Proficiency
  2. Verified ownership of QA Course Sessions 1-16 (Includes Beyond IRT) 
  3. Prepare 2 Case Reports (upload PDF below)
     a. One brief case report: An interesting or unusual case*
     b. One long case report (use Keating Blum format): Follow patient progress for a minimum of 3 office visits demonstrating the use of the QA Clinical Protocol (Upload PDF below)*
